VERO BEACH — The Visiting Nurse Association (VNA) is pleased to announce Denise Clark, RN, BSN, CHFN, is one of nineteen in the state of Florida to earn the prestigious Certified Heart Failure Nurse certification (CHFN.)
Clark, who joined the VNA in 2006, began as a Case Manager in the Space Coast office in Melbourne. Since 2011, she has been focusing her expertise on congestive heart failure patients and is the VNA’s Cardiac Disease and Telehealth Coordinator.
In her role, Clark manages and creates new and alternative processes to decrease rehospitalization for her patients.
“Denise is passionate and dedicated to providing the best possible care to our patients and their loved ones and is always willing to go the extra mile. She takes pride in her work and is a definite asset to the VNA,” said Mary Jane Davis, Vice President of Home Health.
In earning this certification, Clark demonstrated knowledge and commitment to quality care and confirmed her knowledge, experience, and clinical judgment in diagnosing, treating, and managing VNA patients with heart failure.
“I am so thankful that the VNA gave me the opportunity to obtain this certification. I am able to coach my peers, teach my CHF patients, and head them towards the best possible outcome for them: health at home,” Clark explained.
