Jack A. Smith, 58, died Sept. 16, 2012, at VNA Hospice House in Vero Beach.
He was born in Fayetteville, Ohio, and lived in Vero Beach for 30 years, coming from Ohio.
He worked as a carpenter for various construction companies in Vero Beach.
Survivors include his fiance, Kelli Hardwick, of Vero Beach; son, Mark; daughters, Karen and Nicole; brothers, Philip Smith, Dale Smith and Robin Smith; and sisters, Elsie Pardoe, Joyce Egleston and Peggy Davis.
He was preceded in death by his brother, Ronald Smith.
Arrangements are by Cox Gifford Seawinds Funeral Home and Crematory in Vero Beach.
